Request Appointment

Use the form below to request an appointment. We have nurses on staff to administer pregnancy tests, doulas to help with birthing, and an education staff here to provide you with training and resources.

Our services are provided to you at no cost. You can request an appointment using the above form, and we will contact you about openings for pregnancy tests, STI testing (STD Testing), consultations, or other services such as education and baby care products. Does the father of your baby want to attend your appointment with you? He is welcome to do so. Also, he can arrange to meet with our men’s program coordinator while you are both in the office. He can talk through parenting, signup for Boot Camp for New Dads, access other parenting classes, and even get support from other dads.
